Form Request (not requiring sports clearance)

General form requests that do not require sports clearance, please email the form in pdf format to forms@allstarpeds.com. Please complete all of the patient portions of the form, do not send it blank. If you need the form expedited, note that in your message. 

If you need a PA Child Health Form (daycare/preschool)or PA Physical Exam form completed, please request via the Patient Portal. Within the portal navigate to the Documents section and Request a Document. Check the box for the document requested and then hit Send Document Request.

Please note that there is a $15 fee for all forms not requested at the time of the annual check. Forms will be completed within 3 business days. For expedited service (completed within 24 hours), there will be a $30 charge per form. Our billing department will call for payment, which must be received prior to All Star releasing the requested form.
