How to Register for the Patient Portal (Children under 18yrs): 

  • You will need to provide the office with the email address you wish to use to register. Parents will receive PINs for each child that they wish to register via a welcome email automatically sent after your first appointment
  • Each child will receive their own unique PIN
  • The office staff can resend the PIN emails upon request, please call the office and select option 6


**Please note: Once your child turns 18, they will need to set up their own portal account. If you had an account for your child before he or she turned 18, the child will no longer be visible on your account.


How to Register for the Patient Portal (18yrs and older):

  • The adult patient needs to sign an adult HIPAA form and return to the office
  • Office staff will send the welcome email to the adult patient's email provided on the HIPAA form
  • Adult patient can choose to add a parent proxy to their account (see instructions below)
