Request a Note for Missed School/Daycare

We only be provide school notes for those families for which we have provided care during the illness. 

• Please call our triage line (610-363-1330 , option 2 for nurses) to request a note.
• You will need to request a separate note for each patient and for each episode of illness. Do not combine children or illness episodes on one form.
• Please allow 2 business days (not including weekends) from the time you have contacted our triage nurses until it is uploaded to the portal.​
• ​We recommend that you call your school to inquire about their official policy related to absences and school notes.
