Sports Participation Clearance
If you need a form that requires sports clearance,
please use this link to fill out the All Star sports questionnaire. Payment is completed through this link as well.
Forms that require All Star Sports Questionnaire completion:
- PIAA Sports forms (required for grades 7-12 if particpating in sports)
- Private school form or college form asking if student is cleared for physical education, intramurals, sports or dance. (ex. Malvern Prep, Academy of Notre Dame, The Hill School)
- Pop Warner/Marsh Creek Eagles Football or Cheer
- Camp Forms requesting a provider signature (ex. ESF Camp, Boy Scouts High Adventure)
Your form will be ready within 3 business days unless expedited service (completed within 24 hours) is requested. Expedited service is not available on Saturdays. We will upload the form directly to the portal when completed. If you do not have a portal account, we will call you when the form is ready.
Forms filled out ahead of time for well check visits will continue to be free as long as the completed form is returned to the family at the Well Check visit and the Well Check visit is no more than 5-7 days from the request.
